An adequate programme of routine maintenance was not in place, and records including clinical files were also deemed to be maintained in poor order.

Adequate arrangements were not in place for residents to access all general health services and for their referral to all other health services as required.

The inspection also noted that not all minor complaints were recorded by the approved centre.

The clinical file of one child who had been admitted to the approved centre was inspected.

While provisions were in place to ensure the safety of the child, age-appropriate facilities were not provided as the child was admitted to an adult-approved centre.

Furnishings were found to be unsuitable and did not support residents' independence and comfort while two sets of fire doors were found not to function properly when closing.

The approved centre was also not compliant with the Code of Practice on Physical Restraint and the Rule on Seclusion.
